  • High School Requirements

    High School student-athletes must be fully registered with the Athletic Department before the first practice. The following steps/forms must be completed and on-file before an athlete is cleared:
    • All Freshmen must have a physical clearance form on-file with the Athletic Department before the first practice
    • A current physical or alternate year card must be on-file for Sophomores-Seniors/ transfer students
    • Valid examination dates are on or after April 1, 2022 (PDF versions are below) 
    • Complete the HCS Medical Emergency Form 
    • Submit Participation Fee- $75/child/sport ($300 family maximum) *Golf $150/child 
    • Complete all required forms
    • Meet all other eligibility requirements (contained in the Student-Athlete Handbook)

Heritage Christian Schools, Inc. (HCS) is a co-educational, PK-12 program, located in the greater metropolitan Milwaukee area in Wisconsin.  HCS operates as a non-stock, non-profit corporation under Sec. 501(c)(3). HCS is an independent Christian school with a self-perpetuating board of directors.
